What We Provide to the Community
Cherished Care provides private, non-medical care navigation and administrative support services for individuals and families who are overwhelmed by complex systems but do not need clinical care.
Many community members struggle not because services don’t exist—but because navigating appointments, benefits, paperwork, transportation, and follow-ups requires time, organization, and advocacy that families often don’t have.
Cherished Care fills this gap by offering hands-on coordination and follow-through, ensuring individuals remain connected, organized, and supported while interacting with healthcare providers, social service agencies, and community resources.
Our Support Services Include:
Community & Daily Living Support
Support with grocery shopping, essential errands, and routine community activities that help individuals maintain consistency, independence, and quality of life.
Appointment & Task Coordination
Scheduling assistance, reminders, and tracking important obligations to reduce missed appointments and delays.
Transportation & Appointment
Support
Assistance coordinating and providing transportation to appointments, errands, and essential outings, helping reduce missed obligations and caregiver strain. Transportation support is non-medical and focused on access and organization.
Administrative & Documentation Support
Assistance organizing paperwork, forms, records, and correspondence required by agencies and providers.
Advocacy & Follow-Up Support
Ongoing follow-up support and assistance with communication, tracking next steps, and ensuring action items are completed across service systems.
Family & Caregiver Support
Acting as an organizational bridge so families stay informed and involved without being overwhelmed.
Support with medication routines through reminders, organization assistance, and coordination with families and providers. No medical advice or administration provided.
What We Do Not Provide
Cherished Care does not provide medical, clinical, therapeutic, or legal services.
We do not diagnose, treat, or replace licensed professionals.
Our role is coordination, organization, and support—helping individuals access care and services more effectively while staying informed and organized.
